By downloading, installing, or using the Veriva application ("Service"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, do not use the Service. These Terms constitute a legally binding agreement between you and Veriva LLC ("Veriva," "we," "us," or "our").
Veriva is a personal health and medication management application that helps users track medications, manage dosing schedules, log symptoms, and coordinate care with caregivers. The Service may connect to third-party health systems including Epic MyChart to import prescription data.
The Service is not a substitute for professional medical advice, diagnosis, or treatment. Information provided through Veriva, including drug interaction alerts and missed dose recommendations, is for informational purposes only. Always seek the advice of your physician or other qualified health provider with any questions you may have regarding a medical condition or medication.
The AI health companion feature provides general information only and is not a licensed medical professional. Do not disregard professional medical advice or delay seeking it because of something the AI companion has said.
You must be at least 18 years of age to use Veriva. By using the Service, you represent that you are 18 or older and have the legal capacity to enter into these Terms. If you are using Veriva to manage medications for a minor in your care, you represent that you are the parent or legal guardian of that minor.
You must create an account to use Veriva. You are responsible for maintaining the confidentiality of your account credentials and for all activity that occurs under your account. You agree to notify us immediately of any unauthorized use of your account at hello@veriva.health.
Veriva integrates with Epic MyChart to import prescription data. By connecting your Epic account, you authorize Veriva to access your health records as described in our Privacy Policy. This access is read-only. You may revoke this access at any time. Veriva is not affiliated with, endorsed by, or sponsored by Epic Systems Corporation.
You may grant caregivers access to view your medication information. By adding a caregiver, you consent to sharing your health information with that person. You are solely responsible for choosing who receives caregiver access. You may revoke caregiver access at any time within the app.

Veriva offers both free and paid subscription tiers. Paid features are billed monthly or annually as selected at the time of purchase. Subscriptions automatically renew unless cancelled at least 24 hours before the renewal date. Refunds are handled in accordance with Apple App Store and Google Play Store policies for purchases made through those platforms.
The Service and its original content, features, and functionality are owned by Veriva LLC and are protected by copyright, trademark, and other intellectual property laws. You may not copy, modify, distribute, or create derivative works based on the Service without our express written permission.
THE SERVICE IS PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED. VERIVA DOES NOT WARRANT THAT THE SERVICE WILL BE UNINTERRUPTED, ERROR-FREE, OR COMPLETELY SECURE. YOUR USE OF THE SERVICE IS AT YOUR OWN RISK.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, VERIVA LLC SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF OR RELATED TO YOUR USE OF THE SERVICE, INCLUDING BUT NOT LIMITED TO DAMAGES ARISING FROM MISSED MEDICATION DOSES, DRUG INTERACTIONS, OR RELIANCE ON AI-GENERATED HEALTH INFORMATION.
You agree to indemnify and hold harmless Veriva LLC, its officers, directors, employees, and agents from any claims, damages, losses, or expenses arising from your use of the Service, your violation of these Terms, or your violation of any rights of another party.
These Terms are governed by the laws of the Commonwealth of Virginia, without regard to its conflict of law provisions. Any disputes arising under these Terms shall be resolved in the courts of Fairfax County, Virginia.
We may update these Terms from time to time. We will notify you of material changes via email or in-app notification at least 14 days before the change takes effect. Your continued use of the Service after the effective date constitutes acceptance of the updated Terms.
We may terminate or suspend your account at any time for violation of these Terms. You may terminate your account at any time by contacting us at hello@veriva.health. Upon termination, your right to use the Service ceases immediately.
