Please read these Terms carefully before using Veriva. By creating an account or using Veriva, you agree to be bound by these Terms. If you do not agree, do not use the service.
These Terms of Service ("Terms") constitute a legally binding agreement between you ("User," "you," or "your") and Veriva LLC, a Virginia limited liability company ("Veriva," "we," "us," or "our"), governing your access to and use of the Veriva medication management platform, including the website at veriva.health, the web application at app.veriva.health, and any associated mobile applications (collectively, the "Service").
By accessing or using the Service, you represent that you are at least 18 years of age and have the legal capacity to enter into this agreement. If you are using the Service on behalf of an organization, you represent that you have authority to bind that organization to these Terms.
Veriva is a personal medication management platform that provides tools for tracking medication schedules, logging doses, monitoring refills, checking drug interactions, logging symptoms, and coordinating with caregivers. Veriva also offers integration with Epic FHIR-enabled health systems for prescription import and an AI-powered health companion feature.
The Service is provided on a freemium basis. Certain features are available at no charge; others require a paid Premium subscription as described in Section 5.
Veriva is not a medical device, and is not intended to diagnose, treat, cure, or prevent any disease or medical condition. The Service is a personal organization tool only. Nothing in Veriva constitutes medical advice. Always consult a qualified healthcare professional regarding your medications, health conditions, and treatment decisions.
Drug interaction information provided by Veriva is sourced from publicly available databases and AI-generated summaries. It is provided for informational purposes only and may not be complete, current, or applicable to your specific situation. Do not make any medication decisions based solely on Veriva's interaction data without consulting your pharmacist or physician.
AI-generated responses from the Veriva health companion are not a substitute for professional medical advice. The AI companion may make errors or provide incomplete information. Always verify AI-generated health information with a qualified healthcare provider.
To use Veriva, you must create an account using a valid email address. You are responsible for maintaining the confidentiality of your account credentials and for all activity that occurs under your account. You agree to notify us immediately at hello@veriva.health if you suspect unauthorized use of your account.
You may not create an account on behalf of another person without their explicit consent. One account per person — you may not share your account with others.
Veriva offers two Premium subscription options:
All subscription payments are processed securely by Stripe, Inc. Veriva does not store your credit card information. By subscribing, you authorize Veriva to charge your payment method on a recurring basis until you cancel.
Subscriptions automatically renew at the end of each billing period. You will be charged the then-current subscription price unless you cancel before the renewal date. We will send a reminder email at least 7 days before an annual renewal charge.
We may change subscription prices with at least 30 days' written notice by email. Price changes take effect at your next renewal date. Continued use of the Service after the price change constitutes acceptance of the new price.
Subscription prices do not include applicable taxes. You are responsible for any sales, use, or other taxes applicable to your subscription based on your location.
Annual subscriptions include a 7-day full refund window from the date of purchase. If you are not satisfied with Veriva Premium, contact us at hello@veriva.health within 7 days of your annual subscription purchase and we will issue a full refund, no questions asked.
After the 7-day window, annual subscriptions are non-refundable. If you cancel an annual plan after the refund window, your Premium access continues until the end of your current billing period.
Monthly subscriptions are non-refundable. You may cancel at any time and your Premium access will continue until the end of your current billing period. We do not issue pro-rated refunds for unused days.
In cases of verified technical failure preventing access to the Service, we reserve the right to issue refunds or credits at our sole discretion. Contact hello@veriva.health to report service issues.
Veriva's free tier is provided without a time limit. We reserve the right to modify the features included in the free tier with 30 days' notice. We will never charge you for features you were using for free without your explicit consent to a paid upgrade.
You agree to use Veriva only for lawful purposes and in accordance with these Terms. You agree not to:
You retain ownership of all personal health data you enter into Veriva. We do not sell your personal or health data to third parties. Our use of your data is governed by our Privacy Policy, which is incorporated into these Terms by reference.
You may export or delete your data at any time. Upon account deletion, your personal data will be removed from our active systems within 30 days, subject to retention obligations required by law.
You grant Veriva a limited, non-exclusive license to store, process, and display your data solely for the purpose of providing the Service to you.
Veriva is not a Covered Entity or Business Associate under the Health Insurance Portability and Accountability Act (HIPAA). Health information you voluntarily enter into Veriva is protected by our Privacy Policy and the security measures described therein, but is not subject to HIPAA protections. Do not use Veriva as a substitute for HIPAA-compliant health records management.
Epic FHIR integration uses OAuth 2.0 and your Epic credentials are never transmitted to or stored by Veriva's servers. Prescription data imported via Epic is stored in your Veriva account and governed by this agreement and our Privacy Policy.
The Service, including all software, designs, text, graphics, and other content created by Veriva, is owned by Veriva LLC and protected by intellectual property laws. "Veriva" and the Veriva logo are trademarks of Veriva LLC. You may not use Veriva's trademarks without prior written permission.
Nothing in these Terms transfers ownership of any intellectual property to you.
To the maximum extent permitted by applicable law, Veriva LLC's total liability to you for any claims arising out of or related to these Terms or the Service shall not exceed the greater of (a) the total fees you paid to Veriva in the twelve months preceding the claim, or (b) $10.00.
In no event shall Veriva LLC be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of data, loss of profits, personal injury, or property damage, even if Veriva has been advised of the possibility of such damages.
This limitation applies regardless of the legal theory on which the claim is based, including contract, tort, negligence, strict liability, or otherwise.
Some jurisdictions do not allow the exclusion of certain damages. In such jurisdictions, Veriva's liability is limited to the fullest extent permitted by law.
THE SERVICE IS PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, OR ACCURACY OF INFORMATION.
Veriva does not warrant that the Service will be uninterrupted, error-free, or free of viruses or other harmful components. Veriva does not warrant the accuracy, completeness, or timeliness of any drug interaction data, health information, or AI-generated content provided through the Service.
You agree to indemnify, defend, and hold harmless Veriva LLC and its officers, directors, employees, and agents from any claims, liabilities, damages, losses, and expenses (including reasonable legal fees) arising out of or related to your use of the Service, your violation of these Terms, or your violation of any rights of a third party.
You may cancel your account at any time by contacting us at hello@veriva.health or through account settings in the app. Upon cancellation of a paid subscription, your Premium access continues through the end of your current billing period.
We reserve the right to suspend or terminate your account immediately, without notice, if we determine in our sole discretion that you have violated these Terms, engaged in fraudulent activity, or if required by law. In the event of termination for cause, no refund will be issued.
We may update these Terms from time to time. We will notify you of material changes by email at least 14 days before the changes take effect. Your continued use of the Service after the effective date of the updated Terms constitutes your acceptance of the changes.
If you do not agree to the updated Terms, you must stop using the Service and cancel your subscription before the effective date.
These Terms are governed by the laws of the Commonwealth of Virginia, without regard to its conflict of law principles. Any disputes arising out of or related to these Terms or the Service shall be resolved exclusively in the state or federal courts located in Fairfax County, Virginia. You consent to personal jurisdiction in such courts.
